Edit: have learned that the top # is the wiper assembly part # and the lower is a date code. Does that make sense?

Anyone have a photo of a correct 69 Chevelle SS with the build date of your car?

Agreed, the wiper-motor decals from "Sanpac" are exactly like the originals. I bought a few and they are spot on.
