Default Can anybody ID this fan?

I have this fan, it is NOS with original runs from being dipped, but I am unsure of the application. It has very few markings on it: FRONT, H, and W stamps as can be seen in these pictures. 5 blades, 4 rivets per blade. Any info is greatly appreciated!

Is that the measurement from one bolt hole to another? If so, it is 3.25". Also, the center hole has a diameter of 2 5/8" and the widest diameter of the fan is about 18".

That looks & sounds identical to a #3770262 fan I found awhile ago that fits:
1965-66 series 10-30 trucks w/AC.
1965-68 Impalas w/283 cid.
1964-68 Chevelle-Chevy II w/283 or 307cid.and AC.
1968 Camaros w/S/B.
