I am pretty sure Classic Camaro sold them in the late 80's. I used to have an old catalog somewhere.
The one I have was bought in 1994 from Year One and they rated it #3 quality. It is on the car now and fits decent. Mitch, why did you weld on your deck lid?

I, also, lied about it not having a date code. I forgot about this.. and have no idea what it stands for. Mine does not have the extra hole associated with an over the counter lid, as referenced in CRG. It does have an extra slot, assume, for moving the emblem up with a spoiler ? Interestingly, it has no mounting holes for the emblem and never did have. Maybe it was an used lid from a Firebird when the car was repaired ? Lord knows, it had plenty of dents on the top side, that would not have been related to a right rear collision.
